2. Online Appointment Requests

Submitting a form on this website is a request, not a confirmed appointment. Our office reviews each request and confirms it by email or text. To change or cancel an appointment, please call us at (814) 438-7242. Please give accurate contact information so we can reach you.

3. SMS Terms

Hornaman Chiropractic Center text messaging program:

• What we send: appointment confirmations and, after a completed visit, a one-time request to review us on Google. We do not send marketing or promotional texts, and our texts never include details about your condition or treatment.
• How you agree: by entering your mobile number on our booking forms and submitting the form, you agree to receive these texts at that number. Agreeing is not a condition of receiving care. You can also call the office to book.
• Message frequency: varies, based on your appointments.
• Message and data rates may apply. Charges are set by your mobile carrier and plan.
• Opt out: reply STOP to any text to stop receiving messages. You will receive one confirmation, and we will not text you again unless you ask us to.

• Carriers: wireless carriers are not liable for delayed or undelivered messages.
